General Questions
Please call our main office line at (541) 485-8111 during business hours for any questions about your condition, medication, or physical therapy. If we miss your call, we’ll call you back within one business day.

Emergencies
If you need emergency medical assistance, please call 911.

Cancellations
Please give 48 hours’ notice for appointment cancellations.

Registration Process

When entering the Slocum building, the registration team members will be some of the first faces you’ll see. Our First Floor Greeter will be available to help you find your way to the correct floor for your appointment.

Once on the appropriate floor, a floor Greeter will help coordinate your check-in with an available registration desk. We also have kiosks that allow you to check in, depending on your appointment type. Our check-in team will update your demographic and medical history.

Please be sure to bring your insurance card and driver’s license to your visit as we will scan copies into your electronic medical chart. Once your check-in is complete, the care team will come to the lobby to take you back for your visit.

Scheduling Follow up Appointments

After your visit with the care team, you may be asked to make a follow up appointment. We will help you with that process so that you can leave our office with your follow up plan and next appointment in hand.
