Description

As a Care Coordinator, you play a vital role in ensuring seamless coordination and exceptional patient care experiences. Working within the call center, you will be the primary point of contact for patients seeking orthopedic services. The Care Coordinator is responsible for the resolution of patient inquiries through multiple channels such as: incoming calls, voicemails, and the medication refill line. This is a fast-paced, high-volume call center that focuses on a positive experience for each patient.  This position works M-F, 8AM to 5PM on-site.

  • Answer phones and greet incoming calls in a professional and courteous voice and manner.
  • Ensures that a valid “Authorization to Communicate” is in effect when speaking about a patient to someone other than the patient.
  • Follows HIPAA guidelines and maintains confidentiality for all patient and practice information.
  • Triage calls and direct calls as necessary to appropriate Slocum department.
  • Mange, return and document on all non-urgent calls from the dedicated voicemail system for patient care.
  • Manage all prescriptions that are on the Medication Refill line. Refill or send to primary Medical Assistant as indicated.
  • Able to skillfully navigate through EPM and EHR in Next Gen, and outside health information systems.
  • Assist the clinic Medical Assistant teams in facilitating questions for patients.
  • Enter orders as per protocol for the clinical teams.
  • Supports a coordinated workflow for patients between Triage, Medical Assistants, Physicians, and other Slocum departments.
  • Maintain positive relationships with other departments.
  • Participate in staff meetings and other in-services or training as assigned.
  • Ability to work in a fast-paced environment.
  • Consistently display a positive attitude and flexibility in changing situations.
  • Other duties as assigned.

Requirements

  • Certified Medical Assistant
  • Knowledge of orthopedic terminology required.
  • EMR and computer experience required.
  • Medical office and hospital experience desired.
  • High level of knowledge, experience, and skill in Orthopedic care.
  • Clear and effective communication, interpersonal, and conflict resolution skills.
  • Knowledge retention and recall, ability to memorize and recall information.
  • Adaptability and creativity with problem solving and decision-making skills.
  • Organizational ability, able to juggle multiple tasks while talking to callers.
  • Positive role model in demonstrating empathetic patient care and standards of care.
  • Promotes patient education.
  • Ability to type by touch.
